醫療機器學習中的外部驗證與泛化能力
External Validation and Generalisability in Healthcare Machine Learning
A healthcare machine learning model can demonstrate excellent internal performance and still struggle when deployed in a different clinical environment.
This is a generalisability problem.
Why performance changes
Healthcare data reflects the environment in which it was collected. Differences in patient demographics, disease prevalence, clinical guidelines, diagnostic availability, coding practices and treatment pathways can change the relationship between model inputs and outcomes.
A model trained in one tertiary hospital may therefore encounter a different data distribution in another institution.
What should be evaluated?
External validation should assess more than a single performance metric.
Discrimination measures whether the model can distinguish between outcomes. Calibration assesses whether predicted probabilities correspond to observed frequencies. Sensitivity and specificity help evaluate classification behaviour at relevant thresholds.
Performance should also be examined across clinically relevant subgroups and operational settings. A good aggregate AUROC does not establish that predictions are reliable for every population.
Local adaptation requires evidence
Depending on the findings, a model may require recalibration, threshold adjustment or retraining. However, adaptation should be carefully evaluated to avoid improving one population while introducing weaknesses elsewhere.
Where possible, prospective evaluation should establish how the model behaves under real clinical conditions before broad implementation.
Monitor after deployment
Performance can change as patient populations, clinical protocols, documentation and available resources evolve. Deployment monitoring should therefore include data quality, calibration, subgroup performance and clinically relevant outcomes.
For digital health systems intended for international use, local validation is especially important. A model's technical portability does not guarantee clinical portability.
Engineering principle: Validate the model in the environment where its decisions will be used, not only where its training data originated.
If you are developing or deploying healthcare machine learning systems, include external validation and post-deployment monitoring in your implementation strategy.
